Implement a new render pipeline to support improved output.

  • New "deferred" render pipeline
  • Include full block of flagged text
  • Avoid breaking block elements when splitting long reports
  • Add FlagInclusion options to control flagged text
  • Improvements to document builder API

Resolves #79
