This form should only be available to non-ANC and non-children u5 profiles - (#1)

When the user taps on the "Malaria Confirmation" button in the family member's profile (top right dropdown menu), the Malaria Confirmation form opens.

Title of form: Malaria Confirmation

Content: Please follow the exact content in the data dictionary, "CHW 1 - Malaria Confirmation Form" tab.

Style:
Please split the into two sections with the content.
Page 1 - Malaria confirmation
Page 2 - Malaria Prevention

Please use the same format we used in Family #19

QA 5 July Build:

  • {date_hf_malaria_test} needs to be required (highlighted in the data dictionary in hot pink)
  • Should not include next to the {{date_hf_malaria_test} (Date: [time]). Please see screenshot below.

  • {date_hf_malaria_test} skip pattern should occur when {fever_malaria_chw} = Tested positive for malaria in the last 14 days in a health facility, not {malaria_results} = "Positive". (highlighted in the data dictionary in hot pink)
  • {malaria_edu_chw} should be select_multiple - not select_one.
