By invitation of Liga Portuguesa contra as Doenças Reumáticas (Portuguese League Against Rheumatic Diseases), Francisco Nunes, Elsa Oliveira and Ricardo Graça, researchers from Fraunhofer Portugal AICOS, were present at the 26th Fórum de Apoio ao Doente Reumático in Coimbra, a national event organised by the patient association.

The COTIDIANA solution, created to support the care of people with rheumatic conditions, was presented, showcased and tested by different stakeholders (patients, caregivers, doctors, etc) at the. Honest and productive feedback was given praising the solution and how important the role of a digital health is in the current society.

During their one-month internship at AICOS, two students delved into the smartphone data collected from patients involved in laboratory data collections for the COTIDIANA project. Focused on understanding smartphone-based metrics, the interns explored and analyzed keyboard dynamics data from individuals with rheumatic conditions (Osteoarthritis, Rheumatoid Arthritis, and Psoriatic Arthritis).

Their work contributed to the development of algorithms within the COTIDIANA solution. This effort underscores the project's commitment to enabling efficient patient monitoring for clinical care using mobile applications. The students established remarkable relationships between keyboard typing speed (e.g., key pressing time, words/minute) and performance (error rates), providing valuable insights for enhancing the efficacy of the COTIDIANA solution.

Definitely a valuable experience our team wants to repeat.

The COTIDIANA supervisory board meeting, held on the 7th and 9th of November, 2023, proved to be a success. The reviewers, Tariq Osman Andersen, Associate Professor at Copenhagen University, and Alan Bourke, Senior Data Scientist at Novartis, provided positive feedback, commending the project's achievements, technological maturity, and the relevance of the studies conducted by the team. Notably, the team received valuable suggestions for business opportunities, including utilizing COTIDIANA in Pharma's internal decision-making for early trials and as a patient-centric app to enhance drug adherence.

The project's strengths, as highlighted by the reviewers, include an impressive team effort, a high technical achievement with advanced mobile sensing and tracking capabilities, and its readiness for regulatory approval and certification. Suggestions for future development include ensuring that the platform is technologically agnostic, configurability for tailored trials, and prioritizing features based on their maturity. Exciting business prospects were proposed, such as internal decision-making support in clinical trials and promoting patient self-management. The COTIDIANA team was urged to explore these opportunities and conduct a SWOT analysis, considering the competitive landscape, while identifying features that bring tangible value when integrated into clinical practice.

A team of researchers from Fraunhofer Portugal AICOS and NOVA Medical School (CHRC) is conducting a review of the literature on the methods for clinical validation of digital endpoints. The results of this work should help guide researchers in choosing validation methods, identify potential gaps in the literature, and inform the development of novel methods to optimise the clinical validation of digital endpoints.

The need for such work has become apparent as researchers mapped the steps from developing digital endpoints to regulatory approval for its use in clinical trials and care. Clinical validation plays an important role, as the evidence generated through clinical validation is mandatory in applications to regulatory authorities for approval. The lack of clearly defined standards and validation methods is a significant challenge preventing the use of digital endpoints.

The protocol for the scoping review has been published in [JMIR Research Protocols](https://www.researchprotocols.org/2023/1/e47119/), and at the moment, half of the titles and abstracts are nearly screened for inclusion in the scoping review. This yet another contribution of COTIDIANA to improve the research and impact of digital endpoints in the digital health solutions.
