[BYOM] Support Azure-style Streamed Completions

When streaming chat completions, some users report seeing the first chunk contain an empty choices list. As a result, we enter the territory of undefined behavior by attempting to retrieve the first choice. Further, we assume any retrieved completion object will contain a delta.content property. This change tightens-up our expectations, and safe-guards us from oddly-shaped completion chunks.
