As highlighted by @reiffsb in #131 there were several gaps in the documentation. This PR adds new documentation for the
device_exposure,provider,visit_detail,payer_plan_periodandcosttables and updated documentation for other tables in the cdm. This documentation will be further updated and improved as more features are added to the package.
