Created issue templates in an attempt to standardize/add convention to issues. Hopefully will make them more organizable and readable!

I think the issue template idea is really good, should encourage folks to add more details. I left a few comments, the steps to reproduce comment is a light suggestion so feel free to ignore it, but the version info might be a bit more helpful. Thanks for putting this together!

This looks good to me, thanks for doing this @emprzy ! Just suggested some step to help the user.
As a general point, a problem is that issues are underused vers slack messages (direct, fast) so we should be careful not to increase the friction opening one. A bad issue is already better than a good slack DM that only two person can read.
