This PR adds a contribution policy for the FSH VS Code Extension. This includes a
CONTRIBUTING.mdfile and aCODE-OF-CONDUCT.mdfile.One thing to note: I tried to mirror what is in the README when mentioning the name of the project in the CONTRIBUTING document. Let me know if it is too complex and we'd prefer just "FSH VS Code Extension".
