Landstalker: Fix issues on generation

What is this fixing or adding?

This bundles a few fixes for Landstalker world generation:

• Several entrances were ending up with the same name in specific conditions, causing a generation failure (https://discord.com/channels/731205301247803413/1314864280256122880)
• When using a specific short goal ("Reach Kazalt"), endgame locations were excluded but still present and causing accessibility issues (https://discord.com/channels/731205301247803413/1313260966439419965)
• The hint provided by the Lithograph was giving current player name instead of the name of the player whose location contains the hinted item

How was this tested?

This was tested by manually generating ~100 seeds with the previously problematic options set to ensure it was fixed.

Code changes LGTM. Did not test gen.
I think plando might mess with the jewel hints since plando from_pool currently removes the existing item and creates a new item to replace it, so I would recommend looking into that (not in this PR of course)
