Re-do footnotes to work properly on all 3 outputs

Footnotes are process by the markdown into This is another footnote

For PDFs, these can just be left. For ebooks, we have to actually code the link and the end note. There is no footnote functionality. For web pages, we need some sort of JavaScript.
