Supporting Lingering Beech Survey

Notes from lingering ash survey:

When we change survey questions in the app, if we add or remove any questions or change the name of any questions, we must update the website to ensure continued proper functioning. Before we push the new ash survey live, please do the following and then test locally that the full cycle (mobile app to viewing/dowloading observation on website) functions correctly.
