Capture "orphan" submission forms in Patterns

This relates to issue #155. If forms are not connected to a person then it will not be submitted into Patterns. I would like a way to have these "orphan" forms captured in Patterns and then have an easy way to associate it to a person (also, associate it to a test?).

Make "Submissions" page just show orphan submissions
