remove include_deleted from protocol

I removed it from the client in #513, but I didn't have the chance to clean it up all the way.

Hi. Do you perhaps know if it's anyhow possible to get songs that disappeared from playlists (due to label/GPM licensing changes)? By getting, I would just like to get its names and position within the playlist.

Hm, I'm not sure. I don't think I've ever seen an example of that. I suspect they would show up in plentries, but it doesn't look like that call is actually in gmusicapi (it has plentryfeed and I can't remember how they differ). It's in Autoplaylists, though, if that's helpful.
