Add a file save prompt when changes are made and user attempts to exit

When there are unsaved changes to a file and a new file operation is initiated (new, open, exit), prompt the user to save the file as there are unsaved changes.

Also add unsaved change indicator (*) when a new case file is created (untitled.ta3) and first change made (not just on opened and then changed files).
