Add the Submitted monthly report page - CHW

• Clicking a submitted report as a row in the Sent Monthly Tab takes you to the record of the submission
• The title for the submitted report is: “Sent Reports > [Month] [Year]
• The body text is “Submitted by [date - format dd-mm-yyyy] by [username]. @manutarus @paulinembabu Ideally this would actually show the text for the indicators, but what would be the implications on LOE and technically on the device of showing the content that they submitted?

QA March 18:
The issues above look good to me, but there is an issue with the display. You can only see the text - not the numbers. Can we reduce the space for the text (possibly have it wrap more, take up more lines) so we can see the numbers as well? (see below for reference how it looks.

This is currently working. I guess it's the screen size. Let try and resolve that.

@msschroeder I was using a Tablet
