Add a sick child referral form for BA

• Once a user has selected "Refer to facility" and select the type of referral (Add the Refer to Facility - Referral Type #555 ), the user will begin entering referral form information.
• Please convert the XLSForm to JSON. Please check the content in the data dictionary, "CHW 3 - Child Referral Form" tab. The name of the form should be 'Sick child referral'. The XLSForm can be found here
• In the above mockup, the back arrow (or tapping the device back button) takes the user back to the screen they were on before opening the form.
• The SAVE button should be SUBMIT and only becomes highlighted and tappable after all of the required fields in the form are answered.
• There is no mock-up for this yet. Please use a similar style for this as the below screenshot. (Let Roger know if one is needed)
