Add a male family planning registration - CHW

• The family planning registration form is accessible from a profile page top right drop down menu. Only accessible to men where mra = 1 (Forms available to clients based on their profile #6 )
• Once the form is filled, the man is added to the Family planning register when {ecp} = 1
• Form title is "Family planning registration"/"Sajili kwa uzazi wa mpango"
• Follow the exact content from the data dictionary, Tab CHW 2 - FP- Registration - Male
• Native form widget split into 3 screens: Family planning registration info (row 4 -12), Family planning method (row 14 - 22), Family planning method info (row 24 - 35)
