Year of birth : patient transfer pop-up

In patient registration, when the patient is previously registered in CARE, when the user types in the phone number, a pop-up window appears. The pop-up window instructs user to select the correct patient profile and provide the "date of birth" of the patient to authenticate that it is infact the correct patient.

This workflow was revised. Date of birth is changed to only year of birth.

The text in the pop-up window needs to be updated accordingly
