Describe the bug

  • Investigation summary is not generated based on a specific consultation

To Reproduce
Steps to reproduce the behavior:

  1. Go to '...'
  2. Click on '....'
  3. Scroll down to '....'
  4. See error

Expected behavior

  • The investigation summary table should show the facility name as a new row

Facility name to be linked to the investigation record and shown in the UI.
