Password reset functionality is malfunctioning

Describe the bug

the password reset link is not working properly, the link is redirected to an invalid page

To Reproduce
Steps to reproduce the behavior:

1. Go to https://care.ohc.network/
2. Click on forgot password and input the user-id
3. See error

Expected behavior

• the reset link received on the email should work properly
• when we click on send reset link button, there should a notification shown in the page indicating the reset link is send to the email
