When using the ModelChooser in a StreamField in Wagtail versions 2.12 and above:

Items are added and saved successfully via the admin, but when viewed in the admin again after a refresh, the saved items appear without the object name or 'Clear Choice' or 'Choose Another ..' buttons. The id of the object does appear when the item is collapsed.

When downgrading to 2.12.6, the object names appear fine.
