De-couple lightblue-esb-hook from the lightblue application

This hook has become very specific to the needs of esb and can no longer be considered a generic top-level lightblue project. We should spin this off to a more appropriate project location and consider if we want to take the lessons learned and attempt another more generic version of this hook.

Issue is likely dependent on lightblue-platform/lightblue-rest#162

Once that pom dependency in lightblue-rest is changed so it's not required to build would you recommend moving this hook to somewhere else that's closer to the ESB work that we have open sourced? Maybe https://github.com/esbtools/ ?
