I've found the following missing glyphs while transcribing some St. Gall. and Laon manuscripts:

Tollite portas (Laon)

Suggested NABC code: pf2!vi>

VES-tras

Suggested NABC code: clsun1lsa5 (similar to tgsun1lsa5, but without the prepunctum)

Puer natus est (SG)

NO-bis

Suggested NABC code: lspc
(alternatively, provide a glyph for letter "p" without "cedilla").

Additionaly, provide a proper glyph for representing unreadable parts of the manuscript (thinner than a trivirga). Example (from Graduale Novum):
