and

"formsy-react": "^1.1.4",

throws Error when use
<Formsy.Form>
....
</Formsy.Form>

try - i.e. drop the .Form. This is a (subtle) change from previous versions, the new syntax is documented, but the need to change it isn't very well signposted!
