Rework the w2d repository so that it is no longer a submodule of DITA for Publishers:

Move the style2tagmap doctype from d4p doctypes plugin to the word2dita plugin
Move word2dita documentation source from d4p documentation to w2d project
Create plugin zip build process that publishes it to public OT plugin repo
