It worked before the latest update. I ran the package installation recommended in the revised description (the package was already installed) but I still have the extension set to "Off" automatically when I try to install.

The last release only added the GNOME 3.20 compatibility, so it shouldn't have changed anything. Can you run the command I posted here and tell me the results?
Seems for me it was a conflict in folder names. I removed the folder named "Gnome-Shell-Window-Buttons-Extension-11_e.g.o" (dated 2013) and then the shell was willing to install as "[email protected]".
