<h2>How it works</h2>
<p>The simplest way to connect two patients to the same ventilator is to add splitters to the breathing circuit (Neyman & Irvin, 2006). While this design provides air to each patient, the clinician loses all ability to independently monitor and control the parameters of ventilatory support for each patient. It was in response to this design that a joint statement was released by SCCM, AARC, ASA, APSF, AACN, and CHEST on March 26th that discouraged ventilator splitting. </p>
<p>The COVID multivent system, by contrast, gives providers the ability to independently modulate and measure the volume, PIP, and PEEP delivered to each patient using four ball valves (one on the inspiratory and expiratory limb of each breathing circuit).</p>
<p>The COVID MultiVent design relies on four key ideas: </p>
<ol>
<li>Running the ventilator in pressure mode with ball valves regulating the air that goes to each patient. This allows a provider to regulate the volume administered to one patient without affecting the volume to the other patient. This also ensures that changes in lung compliance in one patient do not affect the other patient.</li>
<li>Running the outgoing air from the patient through another ball valve. This allows the provider to regulate how easily the air leaves the lungs and, by extension, how much air is left in the lungs at the end of exhalation. Adjusting the PEEP of one patient does not affect the other patient.</li>
<li>Running the air to each patient through a spirometer (see the assembly guide for more information). This allows the provider to observe how much air each patient is receiving and adjust the corresponding ball valve appropriately. </li>
<li>Attaching an in-line manometer. Reading the manometer at the end of inhalation gives the PIP while reading the manometer at the end of exhalation gives the PEEP. These readings can be used to adjust the appropriate ball valves.</li>

<p><i><b>How does changing lung compliance in one patient affect the other patient using the COVID MultiVent?</b></i></p>
<p>Because the ventilator is run in pressure-control mode, changing lung compliance in one patient does not affect the other patient.</p>
<p><i><b>How accurately does the spirometer measure the volume of air that reaches the patient?</b></i></p>
<p>Our work using test lungs suggests that the spirometer consistently reads 50 mL less than the volume of air administered to the patient, regardless of inspiratory time, positive inspiratory pressure, or patient lung compliance. We recommend running the ventilator in volume-controlled mode with the completed COVID multivent system attached to ensure that the system has no significant leaks before connecting it to patients. </p>
