Adding a new vendor doesn't work

Required sections are as follows (mentioned with a green dot). If those areas are not filled in, data won't be saved.

In the risks table, Likelihood is one of "Rare, Unlikely, Possible, Likely and Almost certain"
Impact is one of "Negligible, Minor, Moderate, Major and Critical"
Risk severity is one of Low, Medium, High and Critical.
