Allow non-SMART usecase for development

The app looks fantastic but setting up a SMART environment is a bit of a high bar when you're just doing local development. It would be really useful if the app supported writing back to a FHIR server (similar to how lforms does) w/o a smart setup in case the server does not require any authentication.

We are definitely considering implementing some kind of way to write back responses to a non SMART-enabled server down the line. Currently our use case revolves around SMART integration with EHRs but I can certainly see the value in enable writing back to a server without going through the SMART auth process.
