I've taken a look at the team cognitive load assessment form and the form can be useful but it doesn't cover all scope of cognitive load.
Besides, I do thing that cognitive load depends on individuals, and should be sensed during One-One meeting.
My suggestion here is to have an interview template to asses cognitive load adding others topics explained in the book : like surprises, do you think process changes fast enough, do you thing you received too many notif, is the size of your squad appropriate regarding its scope,....
