Add issue forms

We can have nicer UI for issue creation using issue forms.

Describe the solution you’d like

Something like
https://github.com/ionic-team/ionic-framework/issues/new/choose

It has buttons for redirection and forms to be filled.

I feel pretty good with the issue templates that we have. I'd be willing to see what you come up with though.
