I have no idea of any of these are possible, but here are some possible additions:
Aetherblade: Blown up by bomb at start, Hit by laser in final room
Aquatic: Final boss, sucked in, Final boss, mini game (press the correct 1-5)
Mai Trin: Electrostatic Shock bomb over players head. Getting hit by either of the shock attacks
Chaos: Getting mini flux bomb during cats (mid fight) - Not sure if this is already detected.
Cliffside: Picking up hammer. Using hammer on object
Deepstone: Using special action (I know this will be a lot, but it might be nice to see if people are not doing the mechanic)

Molten Furnace:
Nightmare: Siax: not breaking bar during phase?, getting hit by medium AoE that spawns adds
Nightmare: Ensolyss: getting hit by hallucinations after medium AoE
Shattered: Destroying tower with ball, Viirastra: getting bomb, Viirastra: getting hit by large explosion after stun phase.
Sirens Reef: Getting Red or Green mechanic. Getting blown off by wind.
Snowblind: Getting frozen
Solid Ocean: Getting hit by beam (not absorbed), Using crystal on boss
Sunqua: Doing lightning phase mechanic (jumping into cloud), getting focused by Tidal Bargain, Green
Swampland: Picking up Wisp (first, and final rooms)
Thaumanova: Using Special on final boss, Attack causing explosion on goo (hitting goo while shield phase), Final boss - Focus with blue beam
Twilight:

Underground: Doing mid puzzle successful (bomb/laser), focus on final boss
Urban Battleground:
Volcanic: Use rock to break shield
And maybe a few more for raids:
Slothasor: eating blue mushroom
Cairn: Getting Agony
Samarog: Lock and Key mechanic (Inevitable Betrayal), Lock and Key failed
Mursaat: Using dispell, claim, protect
Strikes:
Boneskinner: Hit by Grasp, Hit by Wind
Cold War: Hit by trucks, Hit by flame walls
Fraenir: Hit by ice wave, Fixated,
Forging Steal:
Shiverpeaks:
Voice & Claw: Hit by red Pie
Whisper: Getting Green, Getting chains
Anyway, just some ideas that would help me out on training and not missing mechanics.
