[DADMINUI-46] New create table screen

The current Create Table interface is kind of clunky and has some blemishes.

A cool idea: make an interface to allow simple building of relational structures instead of just one-off tables. e.g. you can create a column called group_id, then click a button to either define that table or key-link it with an existing table.
