Feedback form for tutoring interactions #6

In the email that is sent to the tutor and the requesting student, there should be links to a feedback form that allows each party to record how it went.

The tutors form might ask about progress, and what was covered.
The student form might ask about how it went and if the tutor was effective.

The student form should probably be anonymous. We may even want to have the results hidden from the tutors.

Here are some specific form items that could be included:

  • What did you review during the session? [short answer]
  • For how long did you meet? multiple choice [0.5, 1.0, 1.5, 2.0, 3.0, 4.0]
  • In your opinion, how effective was the tutors tutoring? scale from 1 to 10.
  • Other comments? [Free response]
