Incorrect error code returned when connection to query handler is lost

When Axon Server loses the connection to a query handler, for instance, because the query handler sends a message that is larger than the maximum message size, it returns an error with error code AXONIQ-4001. The 4000 range is for command handling errors, not for query handling errors.
